Before you start your group onboarding application, it helps to gather key documents and details so the process goes smoothly. You may not need every item listed, and we may request additional documents depending on your situation.

Basic Group Details

  • Official group name

  • Legal entity type (LLC, Corporation, etc.)

  • Date of formation/incorporation

  • Tax ID (EIN)

  • Registered business address

  • Mailing address (if different)

Primary Contact

  • Full name

  • Title/role

  • Email

  • Phone

Ownership & Management

  • List of owners/partners (names, titles, ownership percentages)

  • Contact info for each owner/partner

  • ID for each owner/partner (driver’s license, passport)

Supporting Documents (as applicable)

  • Articles of Incorporation or Organization

  • Operating or Partnership Agreement

  • IRS EIN confirmation letter

  • Business license or registration

  • Proof of address (utility bill, lease, etc.)

Financial Information

  • Bank account details (for direct deposit or billing)

  • Recent bank statement (for verification)

  • Voided check (for ACH setup if needed)

Insurance & Compliance (if applicable)

  • Proof of liability or workers’ comp insurance

  • Compliance certificates or industry permits

Additional Documents (if needed)

Depending on your group’s services or location, you might also need:
